Our wonderful Center is seeking to hire an Independently Contracted Speech Therapist to provide speech and language therapy for approximately 10 hours a month. Excellent job for those who would like to make a little extra money a month.

LSF-Santa Maria Wisdom Center (SMWC) is an Adult Day Health Care center that provides support for the frail elderly and disabled in Santa Barbara and San Luis Obispo counties. We serve infirmed adults, individuals with disabilities and seniors with Alzheimer's and related dementia.

Job Duties
Conduct initial assessment of the participant as a member of the multi-disciplinary team and develop an individual plan of care.
Conduct reassessments every six months and document in participants' health record when indicated.
Provide speech and language therapy to participants according to their treatment plan.
Record notations made after each treatment, specifying method of treatment and reaction of the participant. Narrative notes shall be entered in the participant's health record, when indicated, and shall be signed by the speech therapist.
Progress notes shall be written at least quarterly in the participant's health record and signed by the speech therapist.
Instruct and train other health team personnel and family members in methods of assisting and improving speech disorders.

Job Requirements
Minimum Qualifications: Must be licensed Speech Therapist, licensed to practice in California, Bachelor's degree in Speech Therapy from an accredited university. Master's degree preferred.
Certification in behavioral analysis
Copy of current professional liability insurance.
TB clearance

Qualifications
One year and a basic understanding of meeting the physical, mental, and social needs of the frail elderly, developmentally disabled and participants with Alzheimer's disease.
