We have an exciting opportunity for a Lead Community Support Facilitator. The LCSF is responsible for providing home-based services to adult individuals with special needs. The LCSF works together with family members, other identified support persons, regional center personnel, and other service providers to develop and carry out a person-centered plan.
The LCSF will assist the individual in daily living activities which allow for the individual to remain living independently in their own home. The LCSF also has the responsibility for quality control, training staff and oversight of staff to ensure adherence to company policies within the individual(s) home as well as participates in the mandatory emergency response rotation of being on-call.
We are looking to hire a LCSF to provide supervision to clients/staff in and around the Lompoc area.
ESSENTIAL JOB REQUIREMENTS
- Must have reliable transportation
- Prior case management experience or personal attendant experience
- Valid Driver's License and proof of auto insurance
