Santa Maria Wisdom Center (SMWC) is an Adult Day Health Care center that provides critical support for the frail elderly and disabled in Santa Barbara and San Luis Obispo counties. We serve infirmed adults, individuals with disabilities and seniors with Alzheimer's and related dementia. Santa Maria Wisdom Center offers individuals and families in-home and community based educational and preventative alternatives to institutional care.

SMWC is seeking to hire an on-call Social Worker to coordinate social services for participants and assist prospective participants through the admission process.

ESSENTIAL JOB DUTIES

•Responsible for coordinating, implementing and maintaining a social services program that meets the needs of individual participants and agency outreach requirements.
•Responsible for recruiting participants and assisting and coordinating their enrollment in the ADHC center.
•Coordinate assessments and visits to the center with prospective participants and their family members or responsible parties.
•Develop social service Individual Plan of Care and narrative after initial assessment and maintain every six months for timely reassessment.
•Maintain monthly social services flow sheets on each participant
•Coordinate weekly team meetings with the multi-disciplinary team and family members.
•Coordinate social service counseling with individual participants when appropriate.
•Provide ADHC participants with referral to outside resources
•Supervise social work assistants, aides and student interns.
•Conduct educational programs for staff and participants as need arises.

ESSENTIAL JOB REQUIREMENTS

•Bachelor's degree with two years of experience providing social services in the fields of aging, health or long-term care services.
•Masters in social work preferred
•One to two years experience in community based programs providing services to elderly, developmentally disabled and/or persons with Alzheimer's disease
•Knowledge of physical, mental and social needs of participant's and skill in working with their families
•Spanish Speaker preferred
•Valid Driver's license and current auto insurance
