Older Adult Services

Life Steps Foundation serves the specialized needs of the frail elderly, Alzheimer’s diagnosed and aging, developmentally disabled people at risk of institutionalization. All our cost-effective, long-term aging services allow participants (at or near the poverty level) to age at home in their communities.

Adult Day Health Care (ADHC) - Our Centers offer a comprehensive program keyed to individual needs.　 We provide door-to-door transportation, on-site nursing, medication management, physical and occupational therapy and rehabilitation, social and recreational activities, breakfast and a hot noon meal. Our aim is to prevent, delay and reduce the premature institutionalization of low income, frail elderly and aging adults with disabilities allowing them to remain in their natural supportive living environments as long as possible.

Hope Alzheimer Resource Centers -  We provide direct treatment and care for persons with Alzheimer’s and other forms of dementia, support groups for caregivers, information and education for caregivers and the community at large. We provide reminiscence therapy, reality-orientation group therapy, medication management, transportation, breakfast and a hot noon meal prepared on site.

Family Caregiver Support Respite Program – Each year this program receives Title IIIE, funding allowing us to offer respite services to clients in San Luis Obispo and northern Santa Barbara Counties. Our respite program provides support and relief to a minimum of 40 unpaid caregivers while allowing many elderly and disabled adults to remain in their homes. Aside from the financial assistance they receive from the respite program, our clients benefit from information and referrals to local community support services.

Tender Loving Care (TLC) Home Services – With our Tender Loving Care Home Services we develop a customized plan designed to support you or your loved one in performing tasks vital to enhancing their ability to live in the community. Whether temporarily recuperating from an injury or requiring 24-hour care, our trained staff meets with you to assess your needs and designs a care plan with your input. Every caregiver undergoes a criminal background and reference check and is bonded/insured, and trained in first aid/CPR and emergency response.
