Aging Services

Many elders suffer chronic medical problems that make daily living difficult and make finding help overwhelming.   Whether they live in urban centers or rural areas, the very real danger exists of these elders sinking into isolation, unable to care for their most basic needs.

Life Steps serves the specialized needs of the frail elderly, those diagnosed with Alzheimer's disease, and those who, developmentally disabled and aging, are at risk of institutionalization.  Our cost-effective, long-term care alternatives for people who live at or near the poverty level provide a variety of health, therapeutic, rehabilitative and social services.   All our Aging Services projects allow participants to age in their homes and communities.

ADULT DAY HEALTH CARE Centers (ADHC)

Licensed by the California Department of Aging, the Adult Day Health Care programs are site-based services that serve those who are developmentally disabled and aging and those who have a developmental disability.  The programs provide a full range of therapuetic, recreational and social activities, and extensive case management and medical / nutritional monitoring.  The goal is to preserve the participants' ability to remain in their own homes, living the lifestyle they choose.  Learn more...

COMPREHESIVE SENIOR CARE (CSC)

Comprehensive Senior Care is a fee-for-service geriatric care management program. Staff address each client's expressed needs to optimize their ability to remain in their homes.  Services include help and referrals for personal, legal and financial needs, services access, medical management, crisis intervention, moving, and other assistance defined by the client.  Learn more...

LINKAGES

Linkages helps individuals maintain self-sufficiency by linking them to needed services when they require help managing their lives because of an illness or other disability.  Services include information giving, referrals and service coordination for help with:

  • Transportation
  • Adult day care or in-home attendant care
  • Health, including medical and nutritional needs
  • Housing requirements and home safety modifications
  • Government benefits programs
  • Support and / or relief programs for family

Adults at least 18 years old and at risk of premature institutionalization due to health and evironmental problems are eligible.  Learn more...
