What is Care Management?
Care management consists of assessing needs, developing treatment plans and linking and managing services.  Services include nursing and physical, occupational and speech therapy.  We also offer medical social services, certified home health aides and caregiver services administered in the patient's home by professional health care providers specially trained and experienced in home health care. The services may consist of intermittent visits or extended continuous care.

Because every situation can be unique, the elderly often need organizational help, emotional support and a clear plan of action.   Moreover, our patients' adult children, who may live far away from that patient, need a local liason who can help with minor problems before they become crises.   Care management is of tremendous benefit in these areas.



Getting Started

After your initial telephone contact, a Care Manager will arrange to come to your home to personally discuss your needs and concerns. Read our Pledge of Ethics

Assessment
This assessment interview usually takes 90 minutes.  Areas of evaluation include health and safety; legal and financial issues; medical history; daily living activities; and emotional and psychological functioning. You will receive a 14-page report with recommendations and referrals to community resources that can help.

Care Plan
After the assessment is completed, you and your care manager will develop a plan that outlines who will do what and when.  Having a Care Plan to follow reduces anxiety, anticipates emergencies and provides specific step-by-step problem solving strategies.

Daily finances are often the first area negatively affected by memory loss.   Sorting the mail, writing checks, balancing the bank statement and keeping up with the bills becomes a source of confusion and frustration.

Our Daily Money Management services can be a reassuring answer.  We handle the incoming mail, pay the bills, balance the checkbook, process insurance claims, provide tax information to tax preparers and coordinate caregivers' payroll.

Through the Comprehensive Senior and Linkages Care Management Program, we provide essential links that promote independence and prevent premature and/or unnecessary institutionalization.  We link people to services, thus enhancing self-sufficiency and maximizing each individual's potential.

We accomplish these goals by providing information, coordination and referrals for transportation, adult day care, home safety modification, medical referrals, money management, nutritional services and friendly visits.   The services we provide include in-home or hospital assessment by professional social workers.
